This is most commonly a sync or cable issue. However, if a controller will not connect under any circumstances, even after a reset, and multiple data cables have been tested, it may have an internal fault with its Bluetooth module or battery.

Turn everything off. Unplug the HDMI cable from both the PS4 and the TV, then plug it back in firmly. Try a different HDMI cable and a different HDMI port on your TV. Also, try connecting the PS4 to a completely different TV to rule out a compatibility issue.

Error CE-34305-5 is a direct indicator of a hard disk drive (HDD) failure. While a loose connection is possible, it most often means the drive itself has developed bad sectors or has mechanically failed and needs to be replaced. This is a common issue for aging launch-model PS4s.

Turn the PS4 off completely, unplug the power cord from the wall outlet, and wait for 2-3 minutes. This can sometimes reset a temporary hardware glitch. Plug it back in and attempt to boot.

If re-seating the drive does not work, the HDD has likely failed. Purchase a compatible 2.5-inch SATA laptop hard drive. Physically replace the old drive with the new one. Then, using a PC, download the full PS4 System Software reinstallation file onto a USB drive (in a PS4/UPDATE folder). Boot the PS4 into Safe Mode and choose option 7: 'Initialize PS4 (Reinstall System Software)'.
